ATTENTION SUBSTITUTES: It is imperative that all substitutes sign the Employee/Substitute Verification Report form at all work sites in order to ensure proper pay.
Substitutes who fill in for itinerant/traveling jobs must sign the verification at all sites assigned to that job.
